Enroll by the Deadline

It’s important to understand the enrollment deadlines and enroll for the coverage you want when you are first eligible. Provided you take the required action, coverage begins on your date of hire. Note that enrollment in the Lyra Health Behavioral Health, basic life insurance and the company-paid STD and LTD plans is automatic.

Voluntary life insurance If you enroll within 31 days of your date of hire, you may elect the following amounts of coverage without providing a statement of health:

  • Up to two times your annual base pay, up to $300,000 (up to $150,000 if you are age 70 or over)
  • Up to $50,000 in coverage for your spouse/domestic partner (up to $25,000 if you are age 70 or over)

Note that a statement of health may apply if you enroll at other times

The benefit elections you make when you are first hired generally cannot be changed until the following calendar year. The only exception is if you have a qualified change in status—such as marriage, divorce or the birth or adoption of a child.
